Consent to Use Specific Content By signing this form, you grant Hope Plan permission to use the specific content you provide, which may include but is not limited to personal stories, video recordings, voice recordings, photographs, documents, and other digital materials, for purposes such as testimonials, marketing, educational content, and other promotional activities.
2. Scope of Usage You understand that the content may be used across various platforms, including Hope Plan’s website, social media, brochures, newsletters, and other public or private communications.This consent applies only to the specific materials you approve, and does not give Hope Plan unrestricted rights to your content beyond what is agreed upon.
3. Privacy and Confidentiality Your privacy is of utmost importance to us. Any content shared will be done in a way that respects your privacy and confidentiality. You may specify any restrictions on the use of your materials, such as whether to exclude sensitive personal information or specific portions of your story.
4. No Compensation You understand and agree that no monetary compensation or royalties will be provided for the use of the materials you provide. Your participation and content sharing are voluntary and in the spirit of helping others learn more about preserving their legacy and heritage.
5. Revocation of Consent You may revoke your consent at any time by providing written notice to Hope Plan. Upon receiving such notice, Hope Plan will cease using the approved content for any future materials but may not be able to withdraw previously published or distributed materials.
6. Release of Liability By signing below, you release Hope Plan, its employees, agents, and representatives from any and all liability related to the usage of the materials you have provided. Hope Plan will endeavor to honor all restrictions and approvals, but you understand that inadvertent usage of unauthorized content may occur, for which you agree to release Hope Plan from liability.
7. Agreement to Terms By signing below, you acknowledge that you have read and understood the terms of this Consent and Release Form and agree to be bound by them.
